High Plan:

  • Co-Insurance: 80%/20% (in network); 60%/40% (out of network)

  • Prescription drug card ($10.00 generic $25.00 brand $45.00 non-formulary), after $100 deductible
  • Deductible ($500.00 Network $1,000.00 Out-of-Network doctors)
  • Wellness care (100% paid up to $300.00 for network doctors)

 

Low Plan:

  • $35.00 Doctor Visit co-pay
  • Co-Insurance: 70%/30% (in network); 50%/50% (out of network)

Life Insurance:

  • $10,000 Life insurance benefit – no additional charge

 

Voluntary Life Insurance:
          Salaried Employees Only

  • Additional coverage to basic company paid plan
  • Guarantee issue up to $300,000 per employee,

$50,000 per spouse and $10,000 per child

  • Available to all full time employees

 

Flexible Spending Accounts: 

  • Tax free reimbursements on dependent care expenses or unreimbursed (not covered by insurance) medical expenses.
  • Save as much as $.22 or more on taxes for every $1.00 used in the Flexible Spending Accounts.

 

401(k):

  • Immediate eligibility for enrollment (first of every month)
  • Employer matches $1 for every $1 contributed on the first 3% and $.50 for every $1 on the next 2% (Company will match 4% of the first 5% employee contribution).

Tuition Reimbursement:

  • Covers tuition and related expenses for approved college courses, degree programs, and the G.E.D.
  • Minimum reimbursement per year is $750; maximum reimbursement per year is $2000, depending on the number of years participating in the plan.
