Medical Insurance:
High Plan:
- $25.00 doctor visit co-pay
- Co-Insurance: 80%/20% (in network); 60%/40% (out of network)
- PPO Network providers with BCBS
- Prescription drug card ($10.00 generic $25.00 brand $45.00 non-formulary), after $100 deductible
- Deductible ($500.00 Network $1,000.00 Out-of-Network doctors)
- Wellness care (100% paid up to $300.00 for network doctors)
Low Plan:
- $35.00 Doctor Visit co-pay
- Co-Insurance: 70%/30% (in network); 50%/50% (out of network)
- PPO Network providers with BCBS
- Prescription drug card($10 generic; $25 Brand; $45 Non-formulary) after $100 deductible
- Deductible ($750.00 Network $1500.00 Out-of Network)
- Wellness care (100% paid up to $300.00 for network doctors)
Dental Insurance:
- Voluntary dental plan with lower premiums
- Orthodontics included up to the age of 19
- No referrals necessary
Vision Insurance:
- Voluntary vision plan with lower premiums
- No referral necessary
- Wide network of providers through Spectera insurance
Life Insurance:
- $10,000 Life insurance benefit – no additional charge
Voluntary Life Insurance:
Salaried Employees Only
- Additional coverage to basic company paid plan
- Guarantee issue up to $300,000 per employee,
$50,000 per spouse and $10,000 per child
- Available to all full time employees
Flexible Spending Accounts:
- Tax free reimbursements on dependent care expenses or unreimbursed (not covered by insurance) medical expenses.
- Save as much as $.22 or more on taxes for every $1.00 used in the Flexible Spending Accounts.
401(k):
- Immediate eligibility for enrollment (first of every month)
- Employer matches $1 for every $1 contributed on the first 3% and $.50 for every $1 on the next 2% (Company will match 4% of the first 5% employee contribution).
- Managed by Fidelity Investments
Tuition Reimbursement:
- Covers tuition and related expenses for approved college courses, degree programs, and the G.E.D.
- Minimum reimbursement per year is $750; maximum reimbursement per year is $2000, depending on the number of years participating in the plan.
Holidays:
Thomas Petroleum observes six (6) paid holidays each year, which are usually:
- New Years Day
- Memorial Day
- Independence Day
- Labor Day
- Thanksgiving Day
- Christmas Day
